Registration
The registration form can be found in the Childhood Cancer Survivorship brochure to the right. Advanced registration is required.
Fees:
$10 per parent/teen/young adult survivor (14y and younger – free)
$45 per health professional and other participants
Free babysitting for 6 year and younger (Pre-registration required)
Payment Information:
Please make checks payable to Children’s Cancer Network. No refund within ten days of course.
Please register no later than September 1, 2007.
